Full Care Management means you have access to a dedicated Care Manager who is your main point of contact. Your Care Manager will oversee the delivery of your support services on a day-to-day basis. Their role is to ensure that you are getting the right care and services to suit your situation and support your independent living. Your Care Manager is also there to answer any questions you may have.
Full Care Management is suited to people who want a bit more support and everything organised for them. They want the support of a dedicated Care Manager who can support them as their needs change and ensure that their services match their care needs.
